From Planning to Action: Supporting Integrated, Collaborative Care for People with I/DD

The NC Council on Developmental Disabilities has funded a new project, From Planning to Action: Supporting Integrated, Collaborative Care for People with Intellectual and Developmental Disabilities, that builds on the work of the Medical Health Home initiative.

The new project is a partnership among The Arc of NC, Easter Seals UCP, and the Autism Society of NC, and provides funding for two (2) pilot demonstrations: 

IDD Health Care Consultation to improve the capacity of primary and community health care providers to treat individuals with I/DD through consultative access to medical and clinical professionals with I/DD expertise.
 
And 
 
Disability-Informed Health Care Navigation to address the challenges encountered when individuals and families move between complex health care and disability service systems. 
 
A competitive application process will be used to select the 2 sites for the pilot demonstrations. The RFP is also available at North Carolina Council on Developmental Disabilities.
